If you would like to make a complaint, please fill out the form below. Alternatively, written complaints can be emailed to admin@cqrdgp.com.au, printed and handed to a staff member or mailed to PO Box 368, Biloela QLD 4715. The form below asks whether your complaint is formal. The difference between a formal and an informal complaint is in the way it is processed and the types of procedures which must be followed. You must provide your contact details if you wish to lodge a formal complaint. However, if you do not wish to make a formal complaint, but want us to know how we need to improve our service, you may remain anonymous.
